I agree with Shalom but apart from that, unless your translation is intended for people who don't speak English all that well, there is really no reason why you should not use "Guide for the Perplexed". It is a spot on translation of the literal meaning of the words and there is nothing awkward or cumbersome about it to describe the kind of soul-searching that people looking for significance in their lives hopefully go through at some stage. Rambam had religious answers but perplexity is pretty much universal to thinking people. Maybe you shouldn't agonise over it.

Maybe, but the content of the book is not at all related to Rambam or Judaism, and the term is familiar in Hebrew and less so to an English speaking not-necessarily Jewish audience, which is why I and considering not using it verbatim.

It seems to be that the author deliberately used the expression which is the name of Rambam's book. I would think the same consideration would apply in an English translation.
