Muž, 64 let, má několik let progredující dys- fagii se zakašláváním se při jídle, poslední dobou udává výrazné bolesti při polykání i nádechu. Byl vyšetřen na ORL i gastroente- rologii. Obraz dislokace jícnu na RTG snímku s polykacím aktem nasměroval diagnostiku správným směrem (obr. 1). CT potvrdilo a upřesnilo nález masivních přemosťujících předních osteofytů C4–C6 degenerativní etiologie, které komprimovaly jícen v oblasti Kilianova ústí; menší dorzální osteoprodukce byla bez neurologické symptomatologie (obr. 2, 3).
Případ byl indikován k ablaci spondylotic- kých struktur. Provedli jsme operaci předním přístupem dle Caspara a šetrně odsunuli vý- razně napjatý, ale nepoškozený jícen s nazo- gastrickou sondou. Pomocí frézy byly za RTG kontroly zbroušeny osteofyty v celém roz- sahu (obr. 4). Dekomprese jícnu peroperačně byla uspokojivá.
Výkon i časné pooperační období pro- běhlo bez komplikací, nastala prakticky úplná regrese potíží. Na kontrolním RTG za šest týdnů a 12 měsíců po operaci nebyly zjištěny známky recidivy (obr. 5).

A 64-year old man has had progressive dysphagia with coughing during meals for several years. He has been recently complaining of severe pain during swallowing and inhalation. He has been examined at both the otorhinolaryngology and gastroenterology wards. An X-ray image showing the dislocation of the oesophagus during the action of swallowing has shown what diagnostics should be performed (Fig.1). A CT scan confirmed and specified the diagnosis of massive bridging anterior osteophytes C4-C6 of degenerative etiology, which compressed the oesophagus in the area of Killian’s dehiscence; minor dorsal ossification was without neurological symptoms (Fig.2, 3).
The ablation of spondylotic protrusions was indicated for the case. The operation was performed using Caspar’s anterior approach and the considerably tight but undamaged oesophagus with a nasogastric tube was carefully moved aside. By means of a milling cutter, under X-ray check, the osteophytes were fully removed (Fig.4). The peroperative decompression of the oesophagus was satisfactory.
The operation and early postoperative period were without complications, and there was practically a complete regression of problems. On the checking X-ray six weeks and 12 months after the operation no signs of relapse were found (Fig.5).
