Source text - English The aim of this study was to compare the analgesic activity of tramadol by oral and rectal administration. The secondary outcomes included tolerability, compliance, and opioid-induced adverse effects.
Patients and methods
The study was multicenter, double-blind, double-dummy, controlled within subject in crossover, randomized within center (series of four). The ethics committees of each study center approved the study protocol, and the patients provided written informed consent. Included in the study were 60 patients with cancer pain. Inclusion criteria were: age (18–75 years); moderate to severe cancer pain (more than 4 on a numerical scale from 0 to 10, see below) unresponsive to step one analgesic ladder drugs (non-opioid drugs); and pain of non-neuropathic origin.
Each patient initially received oral tramadol 50 mg (drops), followed by tramadol sustained release 100 mg orally, and placebo rectally, or tramadol 100 mg rectally and placebo orally, twice a day, in a randomized sequence, on each of 3 days. Patients were allowed to take 50 mg of oral tramadol by drops as needed (four doses per day, to a maximum of 400 mg/day, including the basal dose given by the oral or rectal route).
Statistics
The main efficacy analysis was planned on an intent-to-treat population, defined as all randomized patients, including violators, who completed the crossover design. An additional analysis has been performed on patients who completed the first phase of the crossover design. Descriptive analysis was carried out by the usual methods: mean, standard deviation, values on continuous variables, and absolute and relative frequencies for categorical variables. Homogeneity between the sequences of treatment was checked on all randomized patients at baseline by means of an appropriate test, Student’s t or Wilcoxon’s test, for the unpaired data for the main efficacy variables and for accompanying symptoms (quality of sleep, insomnia, vomiting, nausea, confusion, constipation). The study protocol was planned to prove the non-inferiority of rectal versus oral administration on PI and PR collected by NRS, as main efficacy variables.
The analysis was performed in the intention-to-treat population, i.e. those who completed both crossover periods. Ten patients dropped out during the first period (four and six in the oral–rectal and rectal–oral sequence, respectively), and six patients dropped-out in the second phase.
Translation - Spanish El objetivo de este estudio fue comparar la actividad analgésica del tramadol administrado por dos vías diferentes: la oral y la rectal. La tolerabilidad, el cumplimiento y los efectos adversos inducidos por opioides fueron criterios de valoración secundarios .
Pacientes y métodos
El estudio era multicéntrico, doble ciego, de doble placebo, cruzado (cada paciente constituía su propio control) y con aleatorización independiente en cada centro (series de cuatro). El comité ético de cada centro aprobó el protocolo del estudio y los pacientes firmaron un documento de consentimiento informado escrito. En el estudio participaron 60 pacientes con dolor por cáncer. Los criterios de inclusión fueron: de 18 a 75 años de edad; dolor por cáncer de moderado a intenso (más de 4 en una escala numérica del 0 al 10, véase más adelante) sin respuesta a fármacos del escalón 1 de la escalera analgésica (fármacos no opioides); y dolor de origen no neuropático.
Cada paciente recibió, dos veces al día y durante 3 días: tramadol por vía oral (50 mg en forma de gotas seguidos de 100 mg mediante liberación sostenida) y placebo por vía rectal; o bien, 100 mg de tramadol por vía rectal y placebo por vía oral. La secuencia de administración se realizó de forma aleatorizada. Se permitió la administración de 50 mg de tramadol oral en forma de gotas en caso necesario (4 dosis por día, hasta un máximo de 400 mg/día, incluída la dosis basal administrada previamente por vía oral o rectal).
Estadística
Se planificó la realización del análisis principal de eficacia usando la población por intención de tratar, formada por todos los pacientes aleatorizados, incluídos los que incumplieron el protocolo, que completaron el estudio con diseño cruzado. Se realizó un análisis adicional con los pacientes que terminaron la primera fase del estudio cruzado. Para el análisis descriptivo se usaron los métodos habituales: media, desviación estándar, valores de las variables contínuas y frecuencias (absolutas y relativas) de las variables categóricas. Se verificó, mediante una prueba apropiada (t de Student o de Wilcoxon), la homogeneidad entre las dos secuencias de tratamiento en todos los pacientes aleatorizados en el momento de inicio del estudio para los datos no apareados, las variables principales de eficacia y los síntomas acompañantes (calidad del sueño, insomnio, vómitos, náuseas, confusión, estreñimiento).
Se diseñó el protocolo con el fín de demostrar la ausencia de inferioridad de la forma rectal respecto a la oral usando como variables principales de eficacia la intensidad del dolor (ID) y el alivio del dolor (AD) según la escala NRS.
El análisis se realizó usando la población por intención de tratar, es decir con los pacientes que completaron ambos periodos cruzados. En el primer periodo abandonaron el tratamiento 10 pacientes (4 de la secuencia oral-rectal y 6 de la rectal-oral), mientras que en el segundo lo hicieron 6.

Source text - English In the last decade several randomized clinical trials and registries assessed the short- and long-term clinical outcome of sirolimus-eluting stents (SES) and paclitaxel-eluting stents (PES). Although drug-eluting stents (DES) lead to a decrease in angiographic restenosis and target vessel revascularization (TVR) rates compared with bare-metal stents (BMS), DES caused novel safety concerns such as possible higher very-late stent thrombosis rates. The occurrence of stent thrombosis is not merely a result of premature discontinuation of dual antiplatelet therapy but rather a multifactorial problem caused by several detrimental features, including clinical, coronary lesion, and procedural characteristics. The higher likelihood of late stent malapposition after DES implantation, which is associated with very-late stent thrombosis (1 year after stent implantation), could jeopardize the very long-term clinical beneficial value of DES. Also the observation of a possible clinical TVR “catch-up” phenomenon in the DES-population is of concern.
The long-term clinical results of the treatment of “all-comer” percutaneous coronary intervention (PCI) patients without using any exclusion criteria have been described by our group in the RESEARCH (Rapamycin-Eluting Stent Evaluated at Rotterdam Cardiology Hospital) and T-SEARCH (Taxus–Stent Evaluated At Rotterdam Cardiology Hospital) registries. Although DES have shown superior short- and long-term clinical outcome with regard to TVR rates compared with BMS, it remains unknown whether this effect is sustained. Therefore, the purpose of the present report is to investigate the safety and efficacy profile of the unrestricted use of SES and PES versus BMS in de novo patients undergoing PCI at 6-year follow-up.
Definitions and clinical end points.
Procedural success was defined as successful stent deployment and a residual stenosis 30% by visual analysis in the presence of Thrombolysis in Myocardial Infarction flow grade 3 without the occurrence of MACE within 2 days after intervention.
Definite stent thrombosis was defined as angiographically documented thrombus in or within 5 mm of the stent, accompanied by at least 1 of the following (as recommended by the Academic Research Consortium criteria): 1) acute symptoms; 2) ischemic electrocardiographic changes; and 3) typical rise and fall of cardiac markers. Stent thrombosis was categorized into early (within 30 days after stent implantation), late (within 30 days and 1 year after stent implantation), and very-late (after 1 year after stent implantation).
The primary end point was the occurrence of patient-orientated MACE (defined as a composite of all-cause mortality, any myocardial infarction [MI], and TVR).
Translation - Spanish En la última década, en varios registros y ensayos clínicos aleatorizados, se evaluó el resultado clínico a corto y largo plazo de los stents liberadores de sirolimus (SLS) y de paclitaxel (SLP). Aunque los stents liberadores de fármaco (SLF) conllevan una disminución de las reestenosis angiográficas y de las tasas de revascularización de los vasos tratados (RVT) en comparación con los stents metálicos sin recubrimiento (SMSR), los primeros causaron acontecimientos adversos nuevos, como un posible aumento de las tasas de trombosis del stent muy tardía. La aparición de trombosis del stent no es tan solo el resultado de la interrupción precoz del tratamiento antiagregante plaquetario combinado sino más bien un problema multifactorial causado por varios elementos que pueden resultar perjudiciales, como las características clínicas, de la lesión coronaria y de la intervención. La mayor probabilidad de mala aposición tardía tras la colocación de un SLF, la cual está asociada con una trombosis del stent muy tardía (≥ 1 año después de la colocación de este), podría poner en peligro el beneficio clínico aportado por los SLF a muy largo plazo. También se ha observado un posible fenómeno catch-up de RVT en la población con SLF, lo cual es preocupante.
Nuestro grupo ha descrito, en los registros RESEARCH y T-SEARCH (Evaluación de stents liberadores de rapamicina en el Hospital de Cardiología de Rotterdam y Evaluación de stents Taxus en el Hospital de Cardiología de Rotterdam, respectivamente, por sus siglas en inglés), los resultados clínicos a largo plazo del tratamiento de todos los pacientes (all-comer) a los que se realizó una intervención coronaria percutánea (ICP) sin usar ningún criterio de exclusión. A pesar de que los SLF han mostrado mejores resultados clínicos tanto a corto como a largo plazo con respecto a las tasas de RVD en comparación con los SMSR, sigue sin estar claro si este efecto se mantiene de forma prolongada. Por lo tanto, el objetivo del presente informe es investigar los perfiles de seguridad y eficacia del uso no restringido de SLS y SLP en comparación con SMSR en pacientes de novo sometidos a una ICP y con un seguimiento de 6 años de duración.

I am a Biochemist who has more than 12 years' experience in the biomedical research field and has worked in English, Spanish, and Catalan. Apart from human biology and medicine, I've always been fascinated by languages, and that's why several years ago I decided to become a life sciences translator.
