Source text - English Results: Mean age was 60±12 years and 67 (96%) patients had a type 2 diabetes. Fecal elastase was normal (elastase >200 μg/g) in 47 (67%) patients, mildly decreased (100-200 μg/g) in 10 (14%) and severely decreased in 13 (19%). There was a significant association between elastase levels and time of evolution of diabetes (p=0.049) and between lower elastase levels and the presence of a positive steatocrit (p=0.042). No significant association was found between elastase levels and other chronic complications of diabetes such as retinopathy, nephropathy, neuropathy, microangiopathy or with insulin requirement. Conclusions: One third of this group of diabetic patients had decreased levels of fecal elastase, that was associated with the time of evolution of diabetes. Patients with lower levels of elastase have significantly more steatorrhea. Among diabetics it is possible to find a group of patients with non diagnosed chronic pancreatitis.

Source text - Spanish Una de las primeras referencias, respecto a la determinacion de la funcion exocrina del pancreas en diabeticos, fue una experiencia en ninos diabeticos insulinodependientes publicada en 1990, que muestra una disminucion significativa de los niveles de tripsina y elastasa en suero y de quimotripsina en deposiciones de ninos diabeticos comparados con controles3. Posteriormente, otros trabajos han mostrado que aproximadamente 30% de pacientes diabeticos tipo 2 y 50% de diabeticos tipo 1 presenta insuficiencia exocrina, medida por la disminucion de los niveles de elastasa-1 fecal y 60% de este grupo de pacientes, presenta esteatorrea clinicamente significativa4. Aunque hay resultados discordantes, la experiencia mas numerosa sugiere que la presencia de insuficiencia exocrina, se relaciona con el tiempo de evolucion de la diabetes y la necesidad de insulina. Translation - Japanese 膵臓外分泌機能の診断によると、糖尿病患者の主要な病歴の1つは1990年に論文発表されたインスリン依存性糖尿病児であったことである。対照群と比較した場合、糖尿病児には、トリプシンとエラスターゼの血漿中濃度の顕著な低下、およびキモトリプシンの糞便中濃度の低下がみとめられると同論文は示している。これまでも他の研究において糞便中のエラスターゼ-1濃度の低下の測定で、2型糖尿病患者の約30％と1型糖尿病患者の約50％に外分泌機能の低下がみとめられ、1型糖尿病患者群の60％において顕著に臨床的な脂肪便がみとめられた。矛盾する結果は複数あるが、外分泌機能の低下が糖尿病の経時変化と関係し、インスリンの必要性の有無に関与していることは多くの経験で明らかである。栄養状態および過去に罹患したほかの慢性的な合併症の発症との関連性は不明である。
